To complete your Online Consensus Associate, you will need to do/provide the following:
  • Complete the Consensus Associate online Sign-Up form
  • Upload your professional photo. Formats accepted: .jpg, .jpeg, .gif, .png. Image size should be 290 px X 332 px or larger. If you do not have an image readily available, a default (temporary) image will be used until you can provide your profile image.
  • Upload copies of your professional licenses/certificates
